As a courtesy, we are happy to provide all new patients with your benefits as quoted to us by your insurance company. However it is ultimately your responsibility to know your benefits. Upon booking as a new patient, you'll receive an email with intake paperwork where you can include your insurance plan information. This needs to be submitted a minimum of 2 business days prior to your scheduled appointment for insurance verification.
